Monthly donations can be set-up by completing the Pre-Authorized Monthly Form and mailing it along with a void cheque to #115-12975 84th Avenue Surrey BC, V3W 1B3. Each month, a donation will automatically be debited from your bank account.

*Please note, this form is for unrestricted giving. If you wish to support a specific staff member, please write "for the ministry of..." followed by their name on the form.
